Definitie

Terminologie • Verward, somnolentie, suf, delier, coma, verminderd bewustzijn, bewustzijnsdaling, vernauwd bewustzijn, niet aanspreekbaar, buitenwesten, van de wereld, dement, apathisch, encephalopathisch, lethayrgie

Symptoom • Incidentie onderschat • 2% van alle SEH patiënten • 10% van alle opnames, 50% bij ouderen

Bewustzijn • Staat van besef van zichzelf en omgeving. • Is een toestand van de geest die gekenmerkt is door een besef van het eigen ik en de omgeving.

Componenten bewustzijn • Cerebrale perfusie: autoregulatie cerebrale perfusiedruk = MAP-intracraniële druk • Intracraniële druk; wordt bepaald door inhoud; hersenen, liquor en (bloed)vaten. Afvloed liquor spinale systeem en vasoconstrictie/dilatatie • Cerebraal metabolisme: glucose en zuurstof

Bewustzijn • Bepaald door 2 hersengebieden: – ‘’Arousal’’, wekbaarheid (level of consciousness), formatio reticularis – ‘’Awareness’’, besef (content of consciousness), cerebrale cortex

Pathofysiologie • • • • •

Cerebraal oedeem; leverfalen, hypo-osmolaliteit Verstoring in evenwicht neurotransmitters; drugs, medicatie Verhoogde prikkelbaarheid door elektrolytstoornis Verlaagd cellulair metabolisme; voedingsdeficiënties Minder O2-delivery en mitochondriale dysfunctie; exogene toxinen, CO2, cyanide • Verhoogde permeabiliteit bloed-hersen barrière, hierdoor meer systemische toxinen, maar normale plasma inhoud verhoogd in CSF (zoals eiwit).

• Sepsis; microcirculatie is gestoord, DIS, inflammatoire cytokinen, permeabiliteit bloed-hersen barrière verhoogd, verminderde aanwezigheid neurotransmitters • Leverfalen; ammoniak, vrije vetzuren, vrije radicalen, cerebraal oedeem (cytotoxisch en bloed-hersen barrière) • Uremie; neurotransmitters en aminozuren verminderd aanwezig, accumulatie van bijv methylguanidine

• Hypo/ hypernatriëmie; verhoogde excitabiliteit van neuronen. Verward, gedesoriënteerd, agitatie, lethargie, spierzwakte, bewustzijnsdaling en insulten • Hypo/ hypercalciëmie, hypomagnesiemie; slaperig, spierzwakte, hallucinaties, insulten, coma • Hypofosfatemie; slaperig, ataxia, nystagmus, abducens parese

• Hypoglycemie ; toegenomen epinefrine release-> tremor, diaphoresis, bizar gedrag, insulten, coma • HHS, DKA; osmolaliteit, metabole acidose, vrije vetzuren • Wernicke; trias verward, ataxie, nystagmus (maar ook abducens parese). Hypothermie en hypotensie door ontregeling hypothalamus.
